ELIGIBLE SENIOR UNABLE TO PICK UP VOUCHERS IN PERSON: 1) SEND SOMEONE TO THE DISTRIBUTION SITE, A PROXY, as long as the Proxy brings the completed and signed 2023 PROXY FORM with identification - Proxy forms are required at in person pickup and must be picked up in advance at any Active Adult Center or downloaded online prior to arriving at the Distribution site.
